Permit Procedures

 

A. How to Obtain a Permit Application
The Permit Application form is available on this site along with all other required forms. Simply download the Permit Application form and complete. Permit related questions may be submitted to the following address:  realestate@miami-airport.com.

B. Application Process
In order to initiate the process submit the completed Application along with the documents and fees mentioned below:    

  • Per question #23 of the Application (refer to the details provided), a Table of Organization for your company.
  • Per question #24 of the Application, a copy of your Local Business Tax Receipt (formerly know as the Occupational License).
  • A non-refundable application fee in the amount of $1,000. The check should be made payable to the "Miami-Dade Aviation Department."
  • A copy of the service agreement between the applicant and the customer(s) to whom services will be provided. MDAD will not issue a permit to an applicant unless a fully executed copy of the mentioned agreement is submitted. The required service agreement/contract may be brief, but if must provide the following information:

    • The effective date and term of the agreement/contract.
    • A detailed scope of the services to be provided.
    • The airport location(s) where the services will be provided.
    • The agreement must be executed by both parties.

 

  • Insurance Certificate for General Liability and Auto Liability in compliance with the following Miami-Dade County insurance requirements:
     
    (a) Commercial General Liability Insurance:  On a comprehensive basis, including Contractual Liability, products, and completed operations in an amount not less than $1,000,000.00 combined single limit per occurrence for bodily injury and property damage. Miami-Dade County must be included as an Additional Insured with respect to this coverage.
  • (b) Automobile Liability Insurance:  Automobile Liability Insurance, covering  all owned, non-owned, and hired vehicles used by the Permittee in connection with its operations under this Permit in an amount not less than:
    1. $5,000,000 combined single limit per occurrence for bodily injury  and property damage covering vehicles when being used by the Permittee  on the Air Operations Area (“AOA”) of the Airport.

    2. $300,000 combined single limit per occurrence for bodily injury and
    property damage covering vehicles when being used by the Permittee off of the AOA.

    Quality of Insurance Company: The insurance coverages required herein shall include those classifications as listed in the Standard Liability Insurance Manuals, that most nearly reflect the operations of the Permittee under this Permit. All insurance policies required herein shall be issued by companies authorized to do business under the Laws of the State of Florida. The companies must be rated no less than "A-" as to Management, and no less than "VII" as to strength in accordance with the latest edition of "Best's Insurance Guide", published by A.M. Best Company, Inc., or its equivalent as approved by MDAD’s Risk Management Division.

C. Application Submittal Instructions
The application and service agreement(s) along with the $1,000 permit application fee may be submitted in person OR via courier to:

D. Application Review
Permit applications may be reviewed by more than one division of the Aviation Department. Therefore, the application process may take up to three weeks depending on the complexity of the application.

E. Permit Eligibility
Applicants not registered with the State of Florida Division of Corporations or whose status is inactive, or who is in arrears to the County on another contract, or non-compliant with applicable DBD requirements, are not eligible to receive a permit.

F. Permit Package
Once eligibility is determined, the Department will extend a permit offer by sending the applicant the following:

  • Three copies of the form of Permit which must be executed according to the instructions.
  • Miami-Dade County Single Execution Affidavit which must be completed in its entirety, executed, and notarized.
  • Affidavit of Members, Managing Members and Managers of Florida Limited Liability Company (LLC), if applicable.
  • Affidavit of Members, Managing Members and Managers of Non-Florida Limited Liability Company (LLC), if applicable.

These completed and executed documents must be submitted by the designated date accompanied by the required security deposit in the amount of $1,000 in person or via courier to the address provided below:

G. Permit Approval Process
The permits and accompanying documents are reviewed by staff then forwarded to the County Attorney's Office for legal sufficiency. The length of time for this review depends on the completeness of the documents returned to us.

Once the documents are approved for legal sufficiency the package is rounted to executive staff for further review and execution by the Director.

Please note that an individual and entity track record with other County contracts is pertinent to the decision to award or not award the permit. Outstanding compliance issues on other contracts delay the decision to award the permit.
